Affidavit For Applying Indian Passport For Child In India

Affidavit by Indian Nationals for Child's Passport

REQUIREMENTS:

1. The requirement for attestation of affidavit by the High Commission of India has been dispensed with.

2. If One of the child’s parent is in Singapore, send a duly filled and signed annexure D form click here, self-attested Indian Passport copy and IC copy (PR/ EP/WP/ S PASS/DP) to India. The parent in India can apply the child’s passport with the documents.

3. In case both the parents are in Singapore and need to apply Indian Passport for the child in India, fill in the self-declaration form click here.

4. The physical presence of both the parents is required with original Indian Passports, IC (PR/ EP/WP/ S PASS/DP) and copies of the same along with child’s passport copy and authorized family members Indian passport (if any) or Aadhar card copy who is applying the child’s passport.

5. Processing time is 3 to 5 working days. Fee applicable is S$17 in cash only.

6. Personal particulars form (click here) to be filled in by both the parents. Photograph not required.

Note: For emergency services, apply directly at the High Commission of      India with   an additional emergency fee of S$100 in cashProcessing time is 2 working days.

Affidavit by Foreign Nationals for Child's Passport 

1. Original Singapore/ Foreign passport and IC (PR/ EP/WP/ S PASS/DP) of the parent in Singapore is required along with copies.

2. Submit the Self-declaration/Affidavit form notarized by the Singapore notary public (http://www.sal.org.sg/ ) with the following additional clause:

3. “Also undertake that I will not apply for a Singapore/ foreign passport for my child and in the event of acquiring Singapore / foreign Citizenship, I will surrender the Indian Passport within 3 months”.

4. In case of renewal of child’s passport, please include child's current passport details on affidavit form before getting it notarized.

5. Copies of Indian passport of the parent and birth certificate of the child in India.

6. Processing time is 3 to 5 working days. Fee applicable is S$17 in cash only.
